Paperwork Requirements

Students are required to complete four forms to complete the DE. Forms 1, 3, and 4 are listed on GradSphere and include:

  1. The Office of Graduate Studies Designated Emphasis Application. This is used to apply to the DE.
  2. The FTR DE Progress Report, which is completed by the DE Advisor when you meet prior to the QE.
  3. The Qualifying Examination Application form, which certifies that all required coursework, including DE coursework, has been completed. It also identifies the member of the DE affiliated faculty on the qualifying exam committee.
  4. The Designated Emphasis Report - Final Verification, which must be filed with the Office of Graduate Studies when all degree requirements for the designated emphasis have been met.

All forms should be signed by students’ academic advisor and by the DE advisor before being submitted to Graduate Studies for a final signature. It is the responsibility of students to check your transcript and ensure that your participation in the DE is listed. It is also the responsibility of students to notify the DE advisor when their dissertation has been accepted and all requirements for their Ph.D. program have been completed.
